To qualify as a patient, your household income must be at or below 200% of the Federal Poverty Guidelines. See below. You may be asked to provide proof of income.

What to Bring With You

Proof of income (tax return, pay stub, disability statement, etc.), state or federal photo identification. Anybody who is lacking any proof of income will need to go through an interview process with the MOCS staff.
